GP Referral Guide – Fertility Investigations

Introduction

This guide is designed to support General Practitioners in initiating appropriate baseline investigations for patients experiencing fertility concerns. Early investigations help streamline specialist care, avoid duplication, and provide a more complete picture at the first consultation.

Dr Simon Nothman welcomes referrals for both bulk billed ovulation tracking and fertility consultations. Links to referral forms are provided below.

Female Investigations

Preconception Screen

  • BMI (height and weight)
  • Full blood count + iron studies (include thalassaemia screen if indicated)
  • Fasting glucose, electrolytes (EUCs), liver function tests (LFTs), lipid profile
  • Infectious disease screening: Rubella, Varicella, CMV, Syphilis, Hepatitis B & C, HIV
  • Endocrine profile: TSH, Vitamin D, Prolactin
  • Karyotype
  • Anti-Müllerian Hormone (AMH)

Fertility Hormones

  • Day 2–4 of cycle: FSH, LH, Oestradiol
  • If PCOS suspected, also request:
    • Testosterone (total and free)
    • Free Androgen Index (FAI)
    • 17-OHP
    • Androstenedione
    • HbA1c
    • Fasting insulin and HOMA-IR ratio
  • Mid-luteal phase progesterone

Imaging

  • Pelvic ultrasound (request Antral Follicle Count)
  • HyCoSy – particularly if there is a history of pelvic inflammatory disease, pelvic surgery, or symptoms suggestive of endometriosis.

Male Investigations

Preconception Screen

  • Height, weight, BMI
  • Infectious disease screening: Hepatitis B & C, HIV, Syphilis
  • Vitamin D
  • Karyotype

Semen Analysis

  • Standard semen analysis
  • Include DNA fragmentation and antisperm antibodies
  • Ideally performed at Genea Andrology Labs

Referral Forms

Referral Process – Ovulation tracking

  1. Complete and sign the referral form.
  2. Either the GP or patient emails the form to ovulationtracking@genea.com.au or fax it to (02) 9094 2211.
  3. Patients call 1300 117 698 on days 1–3 of their menstrual cycle to commence tracking.
  4. A summary of results is provided to the referring GP at the conclusion of three cycles or earlier if pregnancy occurs.

Referral Process – Fertility

  1. Complete and sign the referral form.
  2. Either the GP or patient emails the form to connect@drsimonnothman.com.au or fax it to (02)  8357 1740.
  3. Patient make a booking by calling 02 8357 1747 or via Book Appointment link.
